[cisco-voip] CM reset removes service mgcpapp

The config generated by CM should include creating the dial-peer.
Troubleshooting:
* download the config file from CM manually. review contents. does it 
include the dial-peer? If yes then router parsing/applying issue. If no 
then CM config generation issue.
* Config file is generated by TFTP service on CM.  restart tftp. problem 
still happen? reproducible= more likely to find a bug.  TFTP reads local 
database to generate config file.  Is db replication working in the 
cluster?  If replication working then either contents of the database 
are wrong (ccmadmin input issue?) or CTFTP is interpreting the 
information from database incorrectly.
