Likely means the busy tone is being played in-band rather than the disconnect with cause User Busy being propagated all the way back from the PSTN.
Check debugs on the voice gateway and CCM traces to see what's going on.

Hello Group
We have a CUCM 7.1(3a) Cluster and some PRI to the PSTN via 3745 Router.
If we call from a internal extension to a busy other internal extension we get a busy signal and on the display it shows "Busy".
If we call the busy extension from an ISDN line we get the busy signal and the ISDN phone shows "Line Busy".
If we call the same busy extension via PSTN we get the busy signal but on the display it shows "Call Proceed".
If we call from an internal extension to a busy external extension on the PSTN we get the busy signal and on the display it shows "Call Proceed".
Why don't we get "Busy" on the display if we call via PSTN?
Has anyone an idea
Thanks Reto
