[cisco-voip] Strange Unity operator problem

I have created 2 store specific callhandlers, operator_x and operator_y.

For all vm subscribers in store x I have changed the caller input to
transfer to call handler operator_x if they press 0, vice versa for
store y.

Here is the problem that occurs sometimes:

PSTN user dials a user at store x, the user does not answer, and the
call is forwarded to voice mail, during the voice mail greeting, the
user press 0 to speak with store x operator, sometimes the call ends
up on operator_y and everyone is very confused.

This is an older Unity 5. Am I missing something here, Im going crazy,
I have checked everything, tested it over and over again and I can
never reproduce the problem, but still it occurs with 5-10 wrong calls
per day!

We took over this system and I dont know 100% whats configured, is
there like a global operator parameter that Im missing or something??
I have disabled the standard operator call handler.
