Sounds like the 69xx phones may not be using XMLDefaults.cnf.xml. Either that or the values in xmldefault are incorrect.  All phones (configured or not) get xmldefault.  Configured phones (and auto registered phones) get a custom SEP<mac>.cnf.xml built.

what do phone console logs show?

are phones remote from CM? any potential for network congestion?  This exacerbates
CSCto31256    69XX phones have 10 min TFTP DL timeout. Upgrade fails over WAN.

It's not happening with all of my handsets even though they're all on the same LAN segment. It's just plain bizzare.
