Looks like you are running into the tool fraud prevention feature added to 15.1(2)T.

In the log:
Jan  3 14:08:48.567: //273/D15C333180C2/CCAPI/cc_process_call_setup_ind:
   >>>>CCAPI handed cid 273 with tag 3 to app "_ManagedAppProcess_TOLLFRAUD_APP"
Jan  3 14:08:48.567: //273/D15C333180C2/CCAPI/ccCallDisconnect:
   Cause Value=21, Tag=0x0, Call Entry(Previous Disconnect Cause=0, Disconnect Cause=0)

You can disable this feature with the following:
voice service voip
 no ip address trusted authenticate
