We notice on the phones under IPV4 the TFTP server option is missing, in CCMAdmin shows Inactive/Rejected. Another phone same vlan/network you can reboot just phone, same dhcp server.
I'm guessing either phone load but or ccm. These phones were TAPd so the BAT entry replaces the SEP entry, BAT entry is Inactive so as not to use DLUs.

Does the phone have a TFTP server (Network config page) and on right
VLAN? Maybe it got moved / plugged into wrong switchport?
Is TFTP running ok on the server (not hung,etc)?
