[cisco-voip] Moving to 10 digit dialing

2 'styles' of dialing:

Digit-by-digit, and en-bloc.

Digit by digit: Dialing offhook on a FXS port, dialing offhook on a IP phone
En-bloc: Receiving digits via H.323/SIP, speed dials, on hook dialing
on IP Phone

En-bloc will match longest digit.  Digit-by-digit will route on first match.
