[cisco-voip] Voicemail intermittently going to 2 different users

I have a scenario where a user1 in location A transferred to location B and has a different extension now. Then user2 transferred to location A and took over user1's old extension. The Unity accounts were deleted and reconfigured for each user upon arrival at their new locations. 

The problem is user1 intermittently receives user2's voice messages. I can verify the issue by dialing *(ext) and about half the time it goes to user1's VM and the other half to user2's VM. We have verified all settings in CM that have to do with any call forwarding, and also in Unity including alternate extensions. Everything looks right. User2 even receives the missed call on his phone, but it will go to user1's VM, again - sometimes.

Has anyone experienced this phenomenon before? Any suggestions before we open a TAC case?
