[cisco-voip] 8945 to Polycom rmx

There are several issues in this area. Most common recommendations seem to be:

1. limit video bw to 384k. larger values create interop failures
2. upgrade 8945 to 9.3 release based to get fix for CSCtj70239    Video cutthrough time is 1120-1200ms 
3. open a TAC case for further investigation
