[cisco-voip] IOS Upgrade Error on 2921 using c2900-universalk9-mz.SPA.151-4.M5.bin

Almost sounds like a bad flash card in the router.

What does dir flash: show anything ?  format flash: ?

Might want to try replacing the flash card.

We are attempting to upgrade an IOS on a Cisco 2921 Router and we get the following error message when trying to download the appropriate IOS update
