[cisco-voip] CCM 9.1 Bulk Adminitration > Insert Phones > All Details

I enabled logging on the bulk provisioning service in CCMService and found line 4 and line 5 had the same numbers.  In Excel I pointed to the wrong column preparing the file;

Error below isn't very helpful.  None of the DNs exist and nothing is in the Unassigned DNs.  It's one 6961 phone.

Does Bulk Admin inserts show up in CCM trace files?  How can I get more details on what CCM was unhappy with?
