I had a similar issue with CUBE that we figured out was due to the
incoming call from the Call Manager coming in on dial-peer 0.
Take a look at
https://supportforums.cisco.com/thread/2045517
for some example debug statements.
You'll want to see the named events to see the DTMF tones, and make
sure that the call isn't being accepted from CUCM on dial-peer 0.
