[cisco-voip] 'buzz' secretary's phone?

On a slightly more serious note, you could just do an external alerter
(Flashing light / Ringing Bell) that rings when he calls the extension
setup for it.
I'm sure the secretary would appreciate a strobe light or a Klaxon going
off.
