[cisco-voip] CUC 9 Unified message accounts BAT

I have used the following CSV file format recently, to enable Single Inbox,
with success.  "Exchange" is the name of the UM Service, and my corporate
email addresses were already populated from the LDAP sync.  I hope this
helps a little.

subscriberAlias,serviceDisplayName,emailAddressUseCorp,enableMbxSynch
aholloway,Exchange,1,1
