Bounce the RIS DB Collector (Realtime Information Database Collector Service).
This usually brings it backup, then logout / login of the admin GUI.
If the issue keeps happening after restarting, dig a little deeper; DNS, NTP ... all the usual suspects.
Outside of a bug, this can be the result of a network / connectivity blip ... etc. Nothing to really worry about unless it keeps happening.

I’ve got a 3 subscriber cluster 11.5.2SU2 with about 3,500 phones. I’ve got a handful of about 300 phones that are working, no problem. I can call them, they can call me. However under Device > Phone I see them not showing an IP address and show None.
