sounds like a bad WSM, however my team w/ my current employer have run
into MANY problems when dealing w/ dual WSM6 (and WSM7 too)

Was this part of an upgrade? (9400m has ISSUES as it is, think you
might want to consider a 9402 or at a minimum 9400z etc...) BUT..

try this
boot tftp x.x.x.x <filename>
wsm boot tftp x.x.x.x <filename>

This will not reload the box, but actually live boot the box.

Also, not sure if you have tried this but
"disable stand"
and then
"enable mod 2"

Just some things I have had to do to limp a dual box back online w/ no
local access and things "wonky"

Good luck..
