Need Help for AA-100

Hello all,

Just received an AA-100. Hooked up the Veriac and brought up the voltage
slowly.

At 115vac, the readings are pin 2=22v , pin 3= 488v , pin 4= 428v , pin
6= -13.6v , pin 7= 22v ;

the schematic shows pin 3= 440v , pin 4= 365v , pin 6= -16v

My voltage from the wall is 125vac ; pin 3 runs up to 525v. I turned the
power off quickly.

Another clue, The 5AR4 was dead as a doorknob on arrival, a white milky
haze on the top of the tube. Was it cooked?

7591's were not included on this unit.

What is my solution?
