Possibly neutralization.  Are you using 6146 / 6146A /
8298 tubes in the final or are you using 6146B / 8298A
/ 6146W tubes?  That is, 6146W tubes with a code date
of manufacture after 1964.

If you are using the 6146B type of tubes, try
replacing with the 6146 / 6146A type.  That should
cure your neutralization problem.  Of course, there
certainly could be something else wrong.  However,
neutralization is the first place to start.
