If the second supply is installed, but does not have AC/DC power
coming into it, you will see an alarm. If you pull the supply out the
alarm will clear (it might turn yellow until you reboot with the
supply removed).

When you boot from the PCMCIA card it should reformat the flash and
lay down a new file system/JUNOS.  Are you seeing any errors during
the boot from PCMCIA process?  Note that one you have booted from
PCMCIA, the router will normally reboot and use the flash, or failing
that, from alternative media (the HD).  From the below it does seem
that you have bad flash as the HD is being used. You may want to open
a support case, as I believe there is a flash format procedure that
*might* resurrect the RE; else you are probably looking at an RMA.

You might try a request system snapshot; this will, in theory, write
the binary environment from the boot device (the HD in your case)
back to the alternative media (which in your case is now the flash
memory. Also, have you tried a reboot specifying the boot device as
the flash?

lab at r5# run request system reboot media ?
Possible completions:
  compact-flash        Standard boot off flash device
  disk                 Boot off hard disk
