[outages] Bandwidth.com VOIP/SIP outage

I saw issues earlier this week when I was doing traceroutes to their NYC
wholesale and DFW wholesale switches. I was going via XO and seeing lots of
packetloss. I moved the outbound traffic to a provider that got to them via
Cogent and things cleared up instantly.
