[outages] Pacnet/Telstra outage in Singapore

At around 2:30pm EDT we had our Pacnet/Telstra transit links in Singapore
flap 5-10 times each and then go down. They came back up a few minutes
later but we didn't get any routes for approx an hour. At the moment, we
have the full v4 table but no v6.
