[outages] Phoenix-IX

We had several customers and an employee out west that all had issues this
morning. It looks like the traffic was getting to HE in LA and then
stopping. Traceroutes are now looking better. We suspected HE was having an
issue between LA and Denver, but no confirmation of that.
