[rbak-nsp] ether-12-port not functioning

Yes i moved the card from slot 2 to 3 for checking.
How to rectify the EEPROM issue? Need to change the whole card or is there any other way to resolve it?

well,
from your log:
Feb 15 21:25:56: %CSM-6-PORT: ethernet 2/12 link state DOWN, admin is DOWN

So if port ID is 2/12 then slot number is 2.
Your printout is from slot 3..

Or I don't undestand something.. did you move the card from slot 2 to 3 ?
If yes, it looks like you have a EEPROM content fault..
