[VoiceOps] +1 368 997 Exchange shows as "Assigned" but not "In Service" (Canada)

At least one of the carriers figured out what the issue was and sent us an
updated termination rate deck. The other 4 are still "trying to figure out"
why calls are being rejected.

Argh, telecommunications!
