My recollection is that the invoice that is sent out notes the cost for 
the entire industry and your company is assigned a contribution factor 
based on that amount. I've had several people freak out when they got 
their bill.  Your contribution factor is calculated based on your 499 
income. Below is a link that explains how they calculate what you owe.
