I have seen this happen once or twice. I just stopped and restarted the
AvNotifierMgr service on the Unity server. Worked like a charm.

Subject: [cisco-voip] Unity MWI after Exchange restart

I was just asked but I have forgotten the answer.

If you're running Unity UM and you reboot your Exchange server. The MWI
lights get screwed up, due to the logging in of the mailboxes by unity.
What service can you restart to get unity logged back into the mailboxes
so the MWI works again?

Thanks

Scott
