Hmmm, interesting.  If you check the cisco-nsp archives, you can see
that I had a similar-looking crash with a VIP2-40 and a PA-2FE-TX.

In my case, two different VIP2-40s and two different PA-2FE-TXs both
worked fine on FastEthernet port 0, but crashed when there was some
amount of traffic on FastEthernet port 1.

As long as I don't use FE port 1, the combo works flawlessly... - so it's
a different problem that has hit you.  As a first try, you could try
re-seating the PA-2FE (as has already been recommended) - this *does* fix
a number of VIP/PA problems.
