You may have noticed these messages in the log

And the IOS versions:
  Slot 2+3 FlexWAN are running 12.2(18)SXD
  Slot 5 sup720 is running 12.2(18)SXD
  Slot 6 sup720 is running 12.2(14)SX1  <-----

IMHO You may want to upgrade the sup in slot 6 to IOS 12.2(18)SXD

You may also want to have a look at:
