It looks like the datasheet has been updated since I last read it. Right at the top it states:
Note: The workaround described in this document is not required for CallManager 4.0 because the maximum number of Calls Per DN is now database configurable. Refer to How Multiple Calls Per Line Work in Cisco CallManager 4.0 for more information on this.
So I guess it's fixed in v4.x but not 3.3(4).

This is normal for CCM 3.3.3 and lower. I believe it is 'fixed' in v3.3.4 and 4.x but I couldn't find anything in the release notes about it.
Essentially, a phone can have only two calls active at once per line. By pressing the transfer button, you are initiating a new call - when you already have two in queue, you can't initiate a third.
They have some way to work around it, but I don't like it:

hi there,
I have a strange problem of ip phones (7905/12/40/60). I have configured
on a line with a DN. The call waiting service is active.
When I receive a phone call, I can transfer it on another phone.
When I receive 2 calls, one hold and the other active, if I try to
transfer one of that (the active), I have that the tranfer soft button is
not usable.
Is this normal?
