-System > Enterprise Parameters
-URL Directories ... is the value field correct for your server(link to xmldirctory)
            note:use of hostname can often be an issue if the CCM isn't DNS registered
-Service > Control Center
    Stop and restart TFTP
-System > Cisco CallManager
    Reset devices to reset phones(force tftp pull for directory path change)
 
I am shooting from the hip here because I don't know what version of CCM you are running. You can also edit you xmldirectory.xml file file found in CCMCIP directory with a text editor. Verify it is pointing to the right location for the directory file. The same goes here for use of IP in place of hostname.
