For anyone following this thread, Problem SOLVED!

As it turns out, IOS 12.2.15t has a bug which prevents outbound callerid
from working between switch type dms100.

Bug ID# CSC dx50282

An upgrade to 12.3.8t solved the problem.

Nothing like two weeks of messing around with configurations only to
discover its a firmware bug.

Thanks to everyone who offered suggestions.
