Have you done a recent upgrade on CCM application or firewall?  I believe 
moving from 3.3.3 to 3.3.5 will give you a different set of udp ports to 
add to acls.

Do you use the QRT tool on the phones?  you can ask people to start 
reporting the problem via QRT so you can summarize more of the pertinent 
details.
