You can use Real Time Monitoring Tool to monitor this but the
application is a bit bulky (Java Application). For something more
lightweight I also use Cacti/RRDTool to graph the number of registered
phones, active phones, and inactive phones from the Cisco CallManager
MIB; the only downside to this is that the information is averaged out
over 5 minute polling intervals.

'actively monitor' is a bit vague. please clarify -
there are monitoring and recording packages to record conversations
there are perfmon counters on CM to show what lines are in use, how
many calls to a specific location, almost any 'how many' question.
there is presence information in CM5.x, In cm 4.x there are shared
lines, CTI Line monitoring, and clients such as Attendant Console and
ARC Console that allow you to check another user's line status.
Locations based call admission control on CM allows CM to patrol the
amount of media bandwidth to a remote location.
The NAM module for the c6k platforms capture statistics and raw data
on signaling and media in the network independent of call control agent.
