[cisco-voip] ccm 4.1.3 MOh connection lost

hello everyone
i need your help for the problem below.
i have 2 different customer and same problem.
I changed moh file by using :\Program
Files\cisco\MOH\DropMOHAudioSourceFilesHere
it runs smoothly except one or 2 times per a day it suddenly stops.
Even if the ip streaming service still runs(i chck from control center ) i stop and restart the service and then again it starts running.
i also checked from RTMTand it displays moh connection lost. But how can it be. MOH file already on ccm and how can be the connection lost. ;pls look at the attachment.
which parameters should i check.
any idea pls
