[cisco-voip] Soft Keys just go away

This is very undesirable. From a design perspective it means all media
streams go through your MTP resources. In a default install the only
MTP resources available are software MTP's hosted out of the IPVMSApp
service running on CM. This has several design constraints:
1. it's software only so only G.711
2. it uses CPU, memory, and network throughput on your CM server. This
places it in direct competition with ccm.exe which provides your
dialtone and processes the keypresses on your phone.
If MTP Required can be avoided, then it should be. If it cannot be
avoided, then it is best to design in hardware MTPs, dedicate a CM
server as an MTP server only doing MTP and not running CM, or dedicate
software MTPs from an IOS device.
You are using up all of your available MTP's and that is causing the
inability to conference/transfer/hold/park/(invoke any supplementary
service).
