[cisco-voip] Music On Hold Problem

Thanks A lot James, Timothy, Chris and the others.

It has worked!!

First, I had problems with MOH server, James helped me configure it
properly. For PSTN Calls, they required IP multcasting which Timothy
has helped me overcome. The setup is now good.
