I think I had the same problem when I upgraded IPCCX 3.5 to 4.0.5. You might modify your Dialing Strings in the Desktop Cisco Administrator and select ‘None’ for the Area_code/Exchange and Exchange Subscriber Code. Double-check the Outgoing calls options as well.

Anyone ever run into a problem where agents cannot blind transfer from CAD but they can directly from their IP phones? The button to complete the transfer does not go active until the other party answers. The agent can reach over and hit transfer on the IP Phone itself though and the blind transfer completes.
We just upgraded to CRS 5 a couple weeks ago, this is one of the lingering minor problems we had. Didn't have any issue with transferring on version 4.
