[cisco-voip] No MOH, just beeps

Tone on hold (ie the beeps) means CCM was not able to allocate an MOH  
server for the specified source, codec, etc.  It can be anything from  
the MOH server not being registered (remember the MOH server runs  
under the IP voice media streaming app service, not MOH translator)  
to a misconfig between multicast/unicast or not having a particular  
codec enabled for MOH.
