[cisco-voip] MAC address for filling out UCSS

I usually email all licensing issues (even if i don't have an open TAC
case) to licensing at cisco.com and typically I get an answer within an
hour, sometimes less, occasionally more. It appears that as long as
you send it using the email address tied to your CCO account they'll
open a case and tie it back to your CCO ID.
I don't know why, but it seems to work much more quickly/easily than
dialing in or even the using the "case open" tool.
