[cisco-voip] Audio not working on 7940

Very simple config not working; 2620xm, CME 4.1, fxo card, 2 7940's.  Dialing extension to extension works fine.  Dialing to fxo and I can't hear anything through the speaker or headset.  Oddly, if I can leave myself a voice-mail on my cell phone voicemail from the 7940 and listen to it from my cell phone.
