The 2 most common mistakes I've seen are
1) The fiber in the tray is not in the same order on both ends
2) I pair needs to be flipped (aka. Both sides are sending on the same pair)
Both these problems can be solved with a continuity test (shine a light)

without knowing your infrastructure and config - have you tested the fibre cores
to ensure light path is okay? what distance is the link? are the ports on thw switch
configured and not shutdown?  are these dual personality ports being used, if so
have you set the media-type? have you verified the SFP are okay - ie tested them in
another switch?
