If you login to CCMUser as the enduser you should be able to look at
device settings for the device profile. From there you should be
able to subscribe to the EM service.
That said with both "Phone Services web pages" and "Phone Services
Subscribe Page" added to the standard CCM Phone Management user group
I can both add and subscribe device profiles to IP Phone services in
6.1.2.1112-2.
If I change "phone services web pages" to read only then I'm seeing
just what you describe. I can subscribe phones but not device profiles.
This looks to be CSCsr45014, introduced in 5.1(3) and fixed in 6.1(3)
and 7.0. As it was deemed a high risk fix it was not included in any
ES.

profile? That’s almost true for me, except extension mobility gets
added to the phone and not the device profile.

Looks like the role is "Phone Service Subscribe Page" and I don't see
that "Standard CCM Phone Management" group has that role.
Remember you can also subscribe services via the CCMUser page. Your
EM userid should have the device profile assigned so try it with
CCMUser.

You need to subscribe the device profile to the extension mobility
service also for it to show up when the user is logged in.
