[cisco-voip] Upgrade from CUCM 4.2.3 to 6.1.3

In CM6.1.2 similar error was caused by username or password incorrect.  
looks liek that has been fixed in 6.1.3.

attempting to change hostname during upgrade, while technically 
supported, rarely goes well.  I suspect your database is not strated.  
login to the CLI and check "utils service list".  Is database running?

I suspect something akin to CSCsq25838.
