[cisco-voip] Unity 5x Phone View but customer delete CCMAdministror in MLA

I'm going to guess that they are using AXL to connect to CUCM and get
device info. So I'd start by looking at the AXL debugs in CUCM to
see what it thinks is going on.

My test from Unity to http://ccm-ip/ccmcip/authenticate.jsp works
great, I get Un-Authroized.
I followed everything below but see other users in NetPro forum are
getting same error but no resolution. Common thread is MLA.
