We just ran into this today as well and bricked 2x7942's.  Here's a lovely TAC article explaining the issue.  In our case we were running 8.3(1) firmware

factory reset should do just that


Symptom:
If Cisco TAC or CA tells a customer to do a factory reset - it may not do so. A recent change for a non-Cisco equipment high visibility customer changed the way factory resets are applied and the alternate tftp server is retained after the reset. This means that if that altserver is no longer applicable, the phone will not recover.


Conditions:
After factory resetting, it does not recover


Workaround:
The only workaround is to power cycle the phone at just the right time after the factory reset has been initiated. However since there is no indication of when this power cycle is required - this renders this unfeasible and there is essentially no work around.

I have a 7941 and was using the reset procedure were you hold the octothorpe key (pound) down when you plug in the power/network cable and wait till you see the led flash across the bottom but they never do, it sits at the headset key and the two line keys flash amber alternately, I then release the pound key and hit 123456789*0# to proceed with the download and it never does keep cycling. What if anything am I missing and is there another way?
