I have seen something like this before. The issue I experienced was the alias no longer matched the account, I had to delete and recreate the account to resolve the issue.

I tried resetting it on the user (just disabling and enabling it), i've also tried, from the PhoneSystem Integration menu, Checking "Force All MWIs Off for this Phone System, then running a MWI Sync for the entire phone system. I still get the same results.
My last ideas was to tell the user to write down any needed voice messages so that I could recreate his account.

I've checked that and they do not. The only thing fishy that I saw for their MWI configuration was that it wasn't inheriting the extension from the user. I set this option, but I still get the same behavior.
Interesting note, I ran a report on CCM (Also running version 7.0.2). In the system reports I ran a report on voicemail and it lists DNs that currently have lit MWIs. The extensions having the problem are not listed as being enabled, but if I look at that specific user in UCon, their MWI is set to On. I assume this is a syncing issue between UCon and CCM with MWIs. Is there a trace for this specifically?

We get this often when one of the Exchange servers goes for a dive. It works as normal except MWI and outcalling stop working until we bounce Unity.
