[cisco-voip] Call goes to MoH?

This one has me stumped.

CM 4.2

User calls customer.

Call goes on for ~25 minutes.

Suddenly the call is disconnected on the far side, but on the user side she
hears our corporate MoH.

She tries the Hold, resume button and continues to just hear our MoH.
 During this hole time the customer has been completely disconnected.

User hangs up and calls the user back.  All seems fine.

problem is this is the second time this has happened.

I'm looking through the CDR's looking for any clues.  Is there something I
should be looking for?  Anyone ever seen anything like this?
