First, do you have any other 7960s around the house? If so, configure them similarly and see if you get the same issue.
If one gives you dialtone and one doesn't, then I'd consider looking at the firmware version on the phone itself. If both have the same issue, I'd factory reset the phone and pull the new firmware again. Confirm that the firmware on the phone is the same firmware that you're serving to it via your tftp-server statements.
If that doesn't give you any helpful information, maybe it's time to start comparing phones that don't have this problem with this problem child watching SCCP debug scroll across your terminal preferably during off-hours since you'll be flooded with a lot of extra information otherwise....however if my memory serves me correctly, you might be able to redirect debug only to a specified phone so long as you provide the MAC.

Upgraded from CME 4.3 to 8.0 on a 2821 with 30 phones. A button (line 5) on the receptionist 7960 which is used for after hours voice mailbox MWI and access. In 4.3 configuration it would get dial-tone when pressed and user would press VM button and retrieve the voice mails. Now the button no longer does anything when pressed, no dial-tone, nothing. All the other non-monitor lines work fine. For reference I've included configuration excerpt for the line (ephone-dn 30) configured for line 5 on ephone 5 is below
Any hints for additional t/s directions would be much appreciated.
