Make sure you have an annunciator configured and also a MTP. They both need to be part of the media resource list associated with the device pool/phones/gateways.

What do they hear? 4.2(3) had annunciator so unless you changed something around that media resource you should be getting a disconnect cause of unallocated/unassigned number sent back to the gateway, and from there along to the telco.
