[cisco-voip] Unity playing Opening Greeting instead of Subscriber Greeting for 1 subscriber

OK...This provided the following additional output:

"11:16:03.193","New Call Trunk=0 Calling#=[3995] Dialed#=[5916]
Forwarded#=[] Reason=[1] Origin=[2]"

So it sees the Dialed#, but that doesn't get filled into the Forwarded# as
it does in other such calls. I am not sure what the Reason and Origin codes
tell me.

dbWalker does not come up with any errors or warnings related to this
mailbox.

Does this additional info tell us anything useful?
